Interdisciplinary Team
LIFE services are authorized, coordinated, and monitored by the LIFE interdisciplinary team. This health care team is composed of the LIFE primary care physician, nurse, social worker, center manager, therapists, dietician, recreational therapist, home health coordinator, and other ancillary program staff. Working together, the LIFE team members assure that care and treatment are provided in a responsive and caring fashion.
